You call it a custom made upper radiator hose.

IIRC, he built it from a stock 91-94 4G63T water hose, then added a sheet metal section so that it could be bolted to the valve cover. Then, added the oval holes for "fanciness".

Personally, I think it looks great too, and I would possibly do the same.
